Registration fee
changes
Due to rising costs (especially CYO fees), the need to
replace ALL uniforms to comply with our new school colors, lack of
success with our fundraising efforts and the growing debt of our
program, we are being forced to raise our registration fees.
Effective May 1, 2007 the changes are as follows:
The cost of each sport for the first child in a
family in that sport will be $60. The charge for the 2nd child
within the same family participating in the same sport is $50 and
$40 for the third and each additional child within the same family
participating in the same sport. Payments made after the
designated deadlines will be assessed a $10 late fee.
In addition, starting with the 2007-08 season all
families involved in our Athletic program will be required to pay a
$40 annual Booster Club fee.
It is hoped that with these changes we will be able to
continue the excellent programs that we have provided in the past.
Our Registration document (Sports_Registration) has been
changed to reflect this new fee structure.
Participation in the St.
Mary's Athletic Program(s) require the completion and registration
of specific information for record keeping and Parish liability.
This page provides nearly all of those documents, in ".pdf"
(Adobe Acrobat 7.0 Reader) format
so that they can be printed and then completed and turned in to the
Information Services Director and Staff for commission, parish and
diocese filing. There are still three (3) documents we are
working with to add to the Volunteer list, the Fingerprint Card
(special form & paper stock), the Diocese VIRTUS Handbook &
Acknowledgement and the new ***continued education booklet &
acknowledgement***. We hope to add these documents very soon.
At this time we do not have the capability to update and submit
online, that is a function we intend to work toward in the future.

What this webpage does provide is the following document links:
- St. Mary's Athlete Registration Form.
This document is required to be completed by every
athlete that participates in our program(s). It identifies
specific information that is used by the Information Services Group
to register the Athlete and provide their coaches with the specific
data needed the file with CYO. This information is also used
for notification of events and schedules.
- Sports Physical - This document is required
to be completed by every athlete that participates in
our program(s). This document is a requirement of CYO.
An athlete may not participate in any CYO program without a
recent (within 12 months) Physical on file! The
document releases CYO from liability due to injuries that may occur
as well as authorization (signature & date), from a licensed
physician, stating that the Athlete is able to participate in our
sports program(s).
- CYO Eligibility Request Form (front &
back). This form is required to be completed and filed by
every Athlete that participates in our sports program(s) but
is not a member of our Parish. This document
requires the authorization of the Parish Pastor, as well as the
normal signatures (athlete, parent & coach). If the athlete
belongs to another parish, the adjoining Pastor must also authorize
the athletes participation.
- Volunteer/Coach Checklist. This
document identifies, and explains, each of the forms that are
required from any person that volunteers to be part of our program,
whether as a coach or other volunteer working with our athletes.
- Volunteer/Coach Application - This document
is required to be completed by every individual who
volunteers to help in our sports program. It identifies
specific information that is used by the Information Services Group
to register these volunteers and also notify them of events and
schedules that require their attention.
- St. Mary's Athletic Commission By-Laws.
These are the rules and guidelines that guide our organization and
the members that are part of the sports program(s).
- CYO By-Laws. These are the rules and
regulations that all members of the Catholic Youth organization are
required to follow in order to be members of their progams.
